Trouble with certs for OSD media (PXE works fine), native mode

I am in need of a walkthrough in how to create the cert needed for physical media OSD. I think that the problem may be that my current cert does not have the subject field filled out but I am really not sure.

I have seen the article and my cert is the right format. Here is the error I get in the CreateTSMedia.log
